What to Bring to Your Appointment

  • Insurance card(s)
  • Driver’s license (or government identification)
  • List of current medications (including dosages)
  • Records from health care provider, when applicable
  • Patient Registration
  • Medical History and Records Release Forms (completed)

Billing and Insurance
